NOTE (2000/03/28): Some people have reported problems printing the Acrobat (.pdf) versions of the forms.
Try the "Print method -> Print as Image" setting in the Print dialog of Acrobat Reader 4.0: it takes a while longer to process and the output quality might not be as good as true .pdf, but it will do the job.

ADDITIONAL NOTE (2000/03/30): The Acrobat (.pdf) versions of the forms have been updated.
They should now behave as originally intended, and should be compatible with Acrobat Reader 3.0.
